How do I schedule an appointment?

There are a few ways to schedule an appointment. You can fill out the contact form on the website, call me at 224-218-5320 or email me at stacy@holubow.com. I will do my best to get back to you within 24 hours. 

How often are appointments?

Most clients attend sessions on a weekly or every other week basis, which is determined between the client and myself. Additional sessions may be offered if needed and clients are encouraged to reach out if they would like to schedule something outside their regular appointments.

How long are appointments? 

Sessions last 50 minutes. 

Do you offer tele-health services?

Yes, I offer both in-person and tele-health therapy appointments to accommodate your preferences and needs. My approach with therapy doesn't change while being remote.

Do you take insurance/ what is the cost of a session?

The practice accepts Blue Cross Blue Shield PPO insurance. For those with other insurance plans or seeking out-of-network payment, I provide a ‘superbill’ for expenses. This allows you to seek possible reimbursement from your insurance company, ensuring accessibility to my services regardless of your insurance coverage.

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. 

Under the law, health care providers are to submit, "good faith estimates", to clients who are not insured or not using their health insurance for health care services.

  • Providers must give you a good faith estimate of how much your healthcare services will cost. You get the estimate when you schedule care at least 3 business days in advance or if you ask for one.

  • You may be able to dispute your bill if it’s at least $400 more than the estimate.

What is your cancellation policy? 

The practice requires 24-hour notice for the cancellation of appointments otherwise the session will be billed at the usual full rate and will not be covered by insurance.
